You will need to work in partnership with our Learning Disability (LD) case management teams, Lincolnshire County Council (LCC)legal department, service providers, GP surgeries, health colleagues, families and advocacy services.</p>\n<p>You will be expected to work within tight timescales, prioritise and work with colleagues to ensure evidence for the Dol’s is gathered within the required timescales. </p>\n<p>Work will include travel across the county. You must have use of a motor vehicle to be able to travel effectively to meet the demands of the role and be able to travel within reasonable timescales. Work mileage claims are paid for travel from the office to appointments.</p>\n<p>Whilst this is a dedicated role within the teams, to undertake specific DoLS work, there may be times that the post holder will be expected to undertake non dedicated DoLS duties, as a Qualified Practitioner, to support the Service, such as undertaking new Adult Care assessments or reviews or responding to emergencies or safeguarding concerns. Whilst we don’t anticipate this would be a regular occurrence, we are looking for Practitioners who would be flexible within the role and respond to the needs of the Service as needed.</p>\n<p><strong>MAIN DUTIES:</strong></p>\n<ul>\n<li>Manage an allocated caseload of clients from across the LD service, living in community supported living placements or in their own/family home.</li>\n<li>Review the current DoLS order, in conjunction with the key worker, to identify the current care and support in place, the restrictions placed upon the individual, the persons capacity to understand their care needs and consent to the care they receive.</li>\n<li>Discuss and agree with all relevant stakeholders whether the care remains in the persons best interests and is the less restrictive option available to meet the person's needs.</li>\n<li>Completing the DoLS application paperwork – DOL11, DOL3, DOL24. Liaise with providers to gather signed care plans, risk assessments, PRN protocol etc, and liaise with the Key Worker to gather up to date Needs Assessments/Care Plans/Risk Assessments. To liaise with GPs to gather medical evidence and liaise with housing providers to gain copies of tenancy agreements where applicable.</li>\n<li>Maintain and update case notes and other records, write reports as required, give evidence in court in relation to care or other proceedings.</li>\n<li>Work with families to understand the purpose and process of DoLS, support families to understand the role and responsibilities of the Rule 1.2 representative and complete referrals to independent advocacy services where appropriate. Communicate with individuals and their families in ways which are appropriate to their needs.</li>\n<li>Work within expected time frames, understanding the legal framework and building effective working relationships with LCC Legal services. </li>\n</ul>\n<p>Our core office hours are 08:45–17:15 (Mon-Thu), 08:45–16:45 (Fri).
