We have an excellent opportunity available for an Occupational Therapist to join our Integrated Mental Health Service, here at Wokingham Borough Council, on a Full Time, permanent basis.
This is an exciting opportunity to join a passionate and dynamic team, playing an integral role as, we collaborate with The Health Trust to transform our services and develop a dedicated social care pathway. This role, along with the Senior Occupational Therapist post, have been created specifically to establish a reablement offer.&nbsp; Reablement provision will focus on delivering short-term, recovery-focused and goal-orientated support to individuals in the community, and on discharge from hospital.&nbsp;&nbsp;
Occupational Therapists will work collaboratively with care providers and Social Care Practitioners, along with voluntary, community and social enterprise organisations to ensure individuals’ needs are met and that personal well-being and choice is central to assessment outcomes.&nbsp; As part of this, we are keen for Occupational Therapists to conduct sensory assessments to support us to better meet the needs of our neurodiverse populations. Occupational Therapists’ skills are valued and recognised as being key in enhancing the new social care pathway and Integrated Mental Health Service.

Key responsibilities:

<li>Hold a mental health caseload, conduct assessments, provide care/recommendations and services for people who have severe and/or enduring mental health difficulties, and physical health co-morbidities</li>
<li>Support Senior Occupational Therapist with the development of an occupation-focussed short-term reablement service</li>
<li>Produce care plans and risk management plans and ensure their effective implementation.</li>
<li>Support care providers to develop skills to better support our population</li>
<li>Identify safeguarding adult and children concerns and implement safeguarding policies and processes.</li>
<li>Maintain high standards of quality in clinical recording, ensuring information is recorded accurately, appropriately and kept up to date</li>
<li>Comply with all service requirements in terms of lone working in the community.</li>
<li>Liaison with other community mental health teams and inpatient services.</li>

Candidate requirements:

<li>Have a degree in Occupational Therapy and be registered with HCPC</li>
