In particular, Kingston has a large Sri Lankan Tamil community and is home to more people from Korea than any other part of the UK.</p><p><strong>About the Service and Team</strong></p><p>The Hospital Team facilitates discharges from Kingston Hospital and all Out of borough hospitals. The team works alongside multiple professionals to complete Discharge to Assess. We provide a 7 day service. We work alongside the Transfer of Care Hub situated in Kingston Hospital and have a daily presence with a member from the team. The Transfer of care Hub consists of multiple professionals working together to facilitate swift and suitable discharges destinations</p><p>The Access Team is the front door to Adult Social Care.</p><p>Our referrals come from our residents and  professionals via the contact centre.</p><p>We  triage all safeguarding concerns to establish whether section 42 is required.</p><p><strong>About the Role and Our Ideal Candidate</strong></p><p>You will work as part of both the access and hospital team</p><p>This will involve: </p><ul>\n<li>The hospital team provides a 7 day service.</li>\n<li>You will be expected to work weekends on a rota basis and will have a day off during the week</li>\n<li>Facilitate timely discharges</li>\n</ul><p><strong>About You</strong></p><ul>\n<li>Have experience working in adult social care/ hospital work</li>\n<li>Have good communication skills</li>\n<li>Have IT knowledge and be able to work independently on IT systems</li>\n</ul><p><strong>Our Offer to You</strong></p><p>As an organisation that values and nurtures talent, we’re committed to helping you fulfil your potential and will offer you a supportive, friendly and collaborative environment to grow and develop your career and skills.
