We help people avoid going into hospital, be as independent as possible after a stay in hospital and prevent people from having to move into a residential home until they really need to.</p>\n<p>This post is part of a multi-disciplinary team, working together with therapists, nurses, social care and medical practitioners to promote independent living, support and plan their return back into the community. You will work with our service users to assist in their rehabilitation and care programmes, which have been developed by our multi-disciplinary team to encourage independence and self-management.</p>\n<p>Working together, our workplace vision and values guide our organisation, and the contribution of our workforce is key to our culture journey. Supported Living Workers will also be expected to follow all the policies and procedures that are in place e.g. safeguarding of vulnerable adult’s procedures and attend regular training.</p>\n<p>The role of Supported Living Worker requires total flexibility to support 25 Service Users living in 15 tenancies across the borough of St Helens.
