To provide information, advice and guidance for referrals that need signposting to other partners and universal services.</li>\n<li>Undertaking strengths-based outcome focused assessments of need including monitoring, coordination, and review of adults with care and support needs.</li>\n<li>Working a duty function role with colleagues that will incorporate a rapid response to individuals in crisis situations to manage needs and risks where appropriate.</li>\n<li>Maintaining effective liaison with the wider team and other Independent and Voluntary Agencies</li>\n<li>Updating and maintaining case file notes and electronic records</li>\n<li>To work in an integrated way with partners including health and colleagues across LLR and locally</li>\n</ul>\n<p>The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS).
