The team delivers antenatal workshops, introducing solid food workshops and are involved in wider campaigns to promote breastfeeding.</p>\n<p>The core service runs from 9.00am-5.00pm Monday to Friday, with some evening work to cover workshops and to meet the needs of the service. Services are delivered from a variety of locations, including clients’ homes, Family Hubs, clinics, community venues and agile office working.</p>\n<p><strong>Do you have excellent people and communication skills, and enjoy working with the public? Do you have a passion for breastfeeding and extensive knowledge of the benefits? We are seeking a motivated and compassionate person to join our team; someone who understands local health issues and the barriers to breastfeeding.</strong></p>\n<p>You must be able to work as part of a team, and be flexible, adaptable and responsive, as this is a service where the needs of service users come first, and no two days are ever the same.</p>\n<p>You will be able to demonstrate excellent communication skills, enthusiasm and commitment, in order to provide the best possible support and advice to all parents. The successful applicant will have an interest in developing their skills working with adults with disabilities who have complex needs and will be responsible for the provision of specialist assessments and follow-on support to those in need of residential or nursing home placement.<br><br>The successful applicant should have a good understanding of complex physical disabilities, mental health conditions in particular dementia diagnoses, and experience in the provision of equipment. The trusted assessor role involves assessing patients on wards and intermediate care settings to facilitate discharge to a care home residence, this can be new placements or returning placements. You will also support those people once discharged from hospital to ensure a smooth transition into their new home. You will play a key role in an integrated team to facilitate timely discharge from hospital with the person being at the centre of the assessment and recommendations made. Knowledge of up to date moving and handling techniques and good IT skills would be an advantage.
