Pre-falls Physical Activity Lead

About the role:
Active Wirral are looking for a Pre-Falls Physical Activity Lead to join the Active Wellbeing team at Wirral Council. The team work across Wirral, supporting residents to make sport and physical activity part of their everyday lives.
You will co-design and implement a pilot Universal Preventative Offer to support older adults to live stronger, healthier and happier lives. &nbsp;&nbsp;
This role will be based with Wirral Council, working in partnership with Public Health, Primary Care Networks and health partners.
About the Candidate:
You will create an evidence based, systemic approach to reducing the number of adults needing to access the falls prevention programme through increasing activity levels of those residents that have a higher rate of falls due to long term health conditions and health inequalities.
The focus will incorporate approaches such as community asset and strengths led approaches to improving health and wellbeing and will aim to focus on communities who experience the greatest inequalities in health.
You will need to display excellent communication skills and be able to work in an agile, multi-disciplinary and compassionate manner to co-produce the offer.&nbsp; You will need to be creative in your thinking and adopt a systemic approach to place-based working to reach agreed key performance indicators. &nbsp;

<li>To monitor the feedback survey referral system, interpreting the appropriate customer information and logging the required customer referrals.</li>
<li>To plan and action customer phone calls, including inputting verbal and written feedback onto the survey system.</li>
<li>To collaborate with the Quality Assurance team to embed customer feedback into QA audits, reports, and improvement initiatives.</li>
<li>To use a variety of digital systems including Microsoft 365 applications and adult social care case management systems, to access customer information, organise customer communications and record feedback.</li>
<li>To identify areas for co-production and co-design within quality assurance workstreams both from ASC professionals and adults and their carers.</li>
<li>To keep records and produce regular written reports/briefing notes and case studies for the Principal Social Worker and Senior management meetings.</li>
<li>Able to interpret and analyse data and information which may be complex and sensitive to different audiences and use their professional judgment to assess potential issues and offer solutions to those issues.</li>

Young Expert - Children's Social Care (Casual Basis)

Job Title:&nbsp;Mental Health Rehabilitation Worker&nbsp;
Location:&nbsp;Based in Catford in a well connected area near Bellingham, Catford, and Catford Bridge stations, with nearby bus stops. Please be kindly aware there is no step free access at this location and some of our other sites.&nbsp;
Salary:&nbsp;£27,000
Shift Pattern:&nbsp;37.5 hours per week, Monday to Sunday on a rota which includes 3 to 4 night shifts a week between 20:00 - 08:30. There may be some flexibility required around these hours as per service requirements.
<h4>About the Role</h4>
We're looking for a Mental Health Rehabilitation Worker to cover the night shift in our service based in Lewisham which offers specialist intensive rehabilitation for people who require more enhanced interventions than that which is offered by commissioned community services. We support residents who live with us for up to nine months and have been discharged from acute wards to avoid admission into inpatient rehabilitation. We support our residents to step down into less supported accommodation to become more independent and integrated into the community.&nbsp;
In this role, you will ensure resident safety, wellbeing, and progress towards their recovery goals. You will support in creating an environment where they feel safe and empowered, and are building relationships with relevant partners and individuals key towards their journey. You will create support plans, plan and hold activities, and further support them to achieve greater independence. The night Shift plays a crucial role in continuing the high quality care provided during the day, ensuring a safe, respectful, and responsive environment for residents. night staff are expected to remain awake, alert, and actively engaged throughout their shift. Sleeping while on duty is strictly prohibited and may lead to disciplinary action, including dismissal for gross misconduct.
Some key responsibilities include:

<li>Providing night support and supervision across two nearby residential properties.</li>
<li>Monitoring residents’ wellbeing, carrying out regular checks, and responding to any incidents or emergencies.</li>

Churchfield offering 24/7 temporary support for those in active addiction and Cherington supporting abstinent residents to reintegrate into the community.&nbsp;

Assessments need to be proactive, preventative, and personal and include support from other professional services where required, to ensure people receive the information and the level of support required for them to regain or maintain their level of independence and to be safe within the community.Provide goal setting / support plans for individual people as appropriate.
