We are seeking an&nbsp;Advanced Practitioner Occupational Therapist&nbsp;to join&nbsp;Brent’s Reablement Team—an integrated service that combines fast-paced responses to&nbsp;Home First&nbsp;discharges with the delivery of reablement and social care services through a short-term intervention model. This is an excellent opportunity for an experienced Occupational Therapist with a background in working with adults with physical disabilities and older people. The successful candidate will play a key role in supporting the team by providing specialist advice, guidance, and supervision to non-qualified staff (OT Assistants). In addition, the practitioner will have the opportunity to work directly with Brent residents in their homes, carrying out OT assessments and delivering reablement interventions.

Shift Patterns: Shifts patterns can include early shifts from 7.30am or late shifts up to 11pm, followed by a sleep in and/or an early shift the following day. Shifts can include sleep in duties, waking nights, weekends and bank holidays and shift patterns can change to meet the needs of the Service Users. There may be occasions to work additional hours to support the service whilst the team are on training, holidays, and sickness.
Examples of shift patterns are 7.30am - 3.00pm, 7.30am -10am and 2.30p,m -10pm. Shift patterns / rota's can include a sleep in shift, examples of times are 10pm to 8 am, or waking night of 10pm to 8 am.
Travel: Our Service Users live across several tenancies in the borough and rely on the flexibility and reliability of our teams to travel across the borough of St Helensto provide the care they need. A drivers licence is advantageous but is not essential.

Social Care Practitioner
We have an exciting opportunity available for a Social Care Practitioner to join our small, friendly and supportive Sensory Needs Service (Adult Social Care) here at Wokingham Borough Council, on a full time, permanent basis.
As a Social Care Practitioner, you will use a strength-based approach to maximise individual’s abilities to live as independently as possible and remain safe in their community. You will work under the supervision of a registered practitioner to undertake direct work with individuals and their carers.
Adult Social Care provides a range of interventions to adults who need extra support to stay healthy, play a part in their community and lead as fulfilling a life as possible.

Key responsibilities:

<li>Manage your own diary and caseload to complete work in agreed time frames.</li>
<li>Complete strength-based reviews of care in line with our statutory duty.</li>
<li>Co-ordinate feedback from a range of sources, working with advocates where necessary.</li>

Candidate requirements:
<ul>
<li>Competent level of IT skills and ability to learn and use adult social care recording system</li>
<li>Excellent verbal and written communication skills – including ability to engage with individuals with a diverse range of communication needs</li>
<li>Ability to relate to individuals from a diverse range of social, economic, and cultural backgrounds</li>
<li>Awareness of the impact of ill health, disability, social disadvantage, and poverty has on and individuals’ wellbeing</li>
<li>Experience of working in a health or social care setting/ or in a customer facing role.</li>
