Our Team This is an exciting opportunity to join an established group as a Personal Advisor within a busy and highly motivated team. Your Contributions:As a Personal Advisor, you will be responsible for assessing and supporting Care Leavers post 18 and supporting them alongside their social worker. You will build working relationships with the young people in order to support them as they transition into adulthood.Within the Personal Advisor role, you will be responsible for managing your own diary around the needs of the young people you work with. You will support young people to navigate health services, education provision and housing services to name but a few. This will include chairing meetings, devising Pathway Plans, attending appointments with young people and working with partner agencies.The role will involve regular travel across the borough and sometimes further afield. Therefore, the post holder must have a driving licence and access to a vehicle. Our Team:The Reablement service provides short term care and support to adults within their own homes within the Halton area. The team focuses on service users strengths to enable them to stay independent and improve their wellbeing. Our team aims to build service users confidence after a stay in hospital, illness or deterioration in their health; preventing re-admission to hospital or a premature move to a care home.The Reablement Service operates 365 days per year. Our care and support worker shift patterns are across a two week rota, working every other weekend. All posts include a mixture of early and late shifts. Early shifts commence at 7.30am and Late shifts finish at 9pm or 11pm.Enhancements are paid for weekends and unsocial hours.
