Adoption West is a regional adoption agency covering six local authorities in the South West. We are looking for a Chair who has a sound understanding and experience of the adoption process, the skills to chair an Adoption Panel and who can contribute to the overall performance of the Adoption Service.Adoption West have three panel Chairs, with each Chair usually chairing 20 panels a year. Our panels have been run virtually since April 2020 with a very occasional face to face panel held if needed. We also hold face to face training days twice a year for all panel members. The fee paid to the Chair is currently £571 per Adoption Panel. The fee includes preparation for and chairing the Panel meeting, quality checking the minutes of the Panel, involvement in Panel members' annual appraisals and contribution toward the 6 monthly reports as well as online attendance at meetings with the Adoption Advisers or AgencyDecision Makers (currently four times a year and twice a year respectively). An additional fee will be paid for contribution towards any Panel training events and attendance at any SWAC (South West Adoption Consortium) meetings. You will undertaking duties under the Care Act 2014 including assessments and reviews, work closely with social workers in the team to carry out capacity assessments, best interest decision-making, safety assessments, and provide interventions to maximise independence and wellbeing for the people in our service. You will work closely with other professionals and agencies and ensure people using our service receive an excellent standard of social care alongside their mental health care. You will work within key legislative frameworks such as the Care Act (2014), The Mental Health Act (1983) and Mental Capacity Act (2005). In this role, you will undertake a number of diverse specialist or technical activities for adults with care and support needs within organisational and legislative frameworks or assessments from other professionals. You will work closely with external partners and providers of social care reviewing care plans and care provision and responding accordingly to any Safeguarding concerns. Work is face paced but never boring, you will use skills that would be gained through formal qualifications and/or considerable relevant work experience. Roles at this level are expected to work unsupervised for much of the time, accessing and assessing information independently and can be undertaken by telephone, face to face in the community, office base or in service users home.Key Responsibilities:<ul><li>To work within the community Mental Health Recovery and Social Care Reviewing team to provided support and intervention for people with severe and enduring mental illness within the remit of adult social care within the integrated team. To work as part of MDT around the person to promote mental wellbeing, reduce and mitigate risk and promote positive outcomes for service uses.</li><li>To work with service users and families/carers to provide advice and support, assess risk and care needs of service users under the Care Act 2014 and S117 MHA to develop a framework for care plans/programme of intervention to meet their needs. To provide support to service users and providers to help maintain packages of support and resolve issues when they arise to avoid breakdown in care.</li><li>To identify Safeguarding concerns and respond appropriately, contributing to both NHS and Local authority Safeguarding enquiries.</li><li>With support of senior’s and social workers carry out Mental Capacity Assessments and Best Interest decision making.</li><li>To undertake a full range of social care and statutory tasks, provide social care knowledge within the integrated team.</li><li>To provide timely review in line with N-Somerset organisational procedures/polices, statutory duties and Avon &amp; Wiltshire Partnership.</li></ul>This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check. We are looking to employ a flexible driver who are as passionate about our service as we. Community Meals drivers are more than a delivery person, they are in some instances the only contact our clients have with another person during their day. As part of the delivery service you will complete a welfare check, for example ensuring that the person is warm during winter months, or has sufficient drinks in warmer weather, reporting any concerns to the Business Support Team. We deliver our service for 363 days of the year, which includes weekends and Bank Holidays, pausing on Christmas Day and New Years Day.
