Our Team:The Reablement service provides short term care and support to adults within their own homes within the Halton area. The team focuses on service users strengths to enable them to stay independent and improve their wellbeing. Our team aims to build service users confidence after a stay in hospital, illness or deterioration in their health; preventing re-admission to hospital or a premature move to a care home.The Reablement Service operates 365 days per year. Our care and support worker shift patterns are across a two week rota, working every other weekend. All posts include a mixture of early and late shifts. Early shifts commence at 7.30am and Late shifts finish at 9pm or 11pm.Enhancements are paid for weekends and unsocial hours. You will receive a car allowance payment monthly in addition to your business mileage. Your Contributions:The Reablement team is looking for caring, hardworking, compassionate and innovative individuals who can ensure that service users care and support plans are provided to a high standard, empowering service users and promoting their independence. Working within a multidisciplinary team, you will develop and maintain effective working relationships with a range of professionals including our therapy and social work teams. Candidates must possess excellent interpersonal skills and have the ability to effectively communicate with and listen to others. Making A Difference as an Assistant Team Manager in Contact Cares
An opportunity has arisen for a driven and innovative Assistant Team Manager to join Integrated Health and Social Care in the Front Door Service within Contact Cares based at Whiston Hospital, Nightingale House. This large and busy multi-disciplinary team and is the single point of access for health and social care referrals. As the team continues to develop, we are looking to recruit a motivated Assistant Team Manager to join our team within the service. The team consists of both qualified and unqualified Social Care, Nursing and Therapy staff. We offer planned and urgent assessments and provide a 2-hour response under Urgent Community Response Criteria.
The Contact Cares team also provide initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention, to reducing hospital admissions and readmissions the work is varied and interesting. The team operate from 8.00 am until 10.00 pm 7 days per week to include Bank Holidays. This post will be shift working to a rota over 7 days.
