We are currently recruiting for a Social Worker within our integrated discharge team to contribute to the delivery of highly effective children and adult services (within Adult Social Care) in an efficient manner. The service is delivered using a strength based, personalised approach.We work across both the acute and non-acute sites across Medway and out of area hospitals. We have been at the forefront of changes in the hospitals discharge to assess model. This has meant that our teamwork with people in an acute setting as well as those in community settings after a period of the time in hospital. This can either be in a health bed, social care step down bed or in the persons’ own home. Team Manager - Assessment

We are seeking a Qualified Social Worker to join the 16+ Service to primarily support unaccompanied asylum-seeking children (UASC) who are children in care to the Local Authority.Applicants will need to have good knowledge and experience of working with, assessing and supporting UASC who are therefore children in care, with full understanding of the Age Assessment, Home Office, Asylum and Children in Care processes and legislations. Applicants will need to demonstrate their experience of working with children in care, UASC, care leavers, adolescents and/or children and families without recourse to public funds, but also a keen interest in working with and supporting young people who have experienced trauma and where English is not their first language is key. Competence in working with the Home Office, associated processes and the age assessment process is essential.
