Community care officer Level 2

Community care officer Level 2

£32,597 - £35,412 per year
Job Introduction

We are seeking Community Care Officers (Level 2) to join our team.

Join a team that put people first! At Lincolnshire Adult Care, we're passionate about helping people live fulfilling, independent lives. We're looking for compassionate, motivated practitioners who share our commitment to person-centred, strengths-based practice and delivering outstanding outcomes for adults across South Lincolnshire.

Whether you're an experienced practitioner or looking to further develop your career in Adult Social Care, you'll join a supportive and forward-thinking service that values collaboration, professional development and innovation.

If you're someone who is curious, reflective, solution-focused and committed to making a genuine difference, we'd love to hear from you.

About the Role

As a Community Care Officer, you'll work with adults who may be experiencing physical disabilities, long-term health conditions, frailty, or other care needs. You will be working with people living in a variety of settings, including their own homes, supported accommodation, residential care and nursing environments.

Working within Lincolnshire's strengths-based Conversation Model, you will:

  • Complete Adult Needs Assessments and reviews under the Care Act 2014
  • Develop strengths-based support plans that focus on individual outcomes
  • Work alongside individuals, carers, families and partner agencies
  • Promote independence, wellbeing and community connections
  • Respond to safeguarding concerns and contribute to safeguarding enquiries
  • Support people through periods of change, crisis and transition
  • Manage a varied and rewarding caseload across your locality

The Area Team focuses on working with adults who require assessments, care planning, and ongoing interventions to meet their care and support needs. This often involves undertaking comprehensive assessments, developing person-centred support plans, and working collaboratively with families, carers, health professionals, and other partner agencies. The team covers Sleaford, Spalding, and the surrounding areas, including the whole of South Holland.

The Review Team focuses on completing planned reviews for people who already have care and support services in place, ensuring that arrangements continue to meet their needs and desired outcomes. The team also supports people following discharge from hospital, helping them adapt to changes in their circumstances and reviewing support where required. The team is based across Grantham, Stamford, Sleaford, and Spalding, providing services throughout those localities.

Currently, we have five vacancies available:

  • 2 full time Community Care Officers – Spalding Area Team
  • 1 full time Community Care Officer – Sleaford Area Team
  • 1 full time Community Care Officer -Stamford Area Team
  • 1 full time Community Care Officer – the office base for the Review Team might be negotiable.

lease mention which post you prefer in the application.

This is a community-based role, and you will need access to a vehicle and be able to travel across the South Lincolnshire area.

At Lincolnshire Adult Care, we believe people are the experts in their own lives.

Our practice is built on meaningful conversations that explore what matters most to individuals, recognising their strengths, aspirations, relationships and community connections. Through our Conversation Model, we work in partnership with people to identify solutions that build independence, resilience and wellbeing.

Rather than focusing solely on services, we focus on helping people make the most of their own abilities, support networks and local communities—ensuring they remain at the centre of every decision.

We are proud of our collaborative culture, where colleagues support one another, share expertise and celebrate success together. Our teams are passionate about delivering high-quality practice and continually improving the experience of adults and carers across South Lincolnshire.
