About the role

As an Advanced Practitioner, you will lead on peer development and practice, supports and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire’s Connected Lives principles to contribute to the ongoing development of the service.

Advanced Practitioners are responsible for delivering high quality statutory social care including recognising safeguarding, delivering assessment, supervision and care management functions as well as developing innovative, creative community solutions.

Advanced Practitioners are pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families, and carers.

There is responsibility for ensuring that all statutory health and social care responsibilities are met and delivered to a high standard including safeguarding, assessment, supervision, and care management functions.

We are re-energising our adult social care model of Connected Lives approach that has a strong ethos of strength-based working, identifying personal goals and ensuring personal, family and community resources are utilised to achieve positive outcomes for people.


Our teams

Transfer of Care Hub
Stevenage
37 hours per week | Permanent

The Transfer of Care Hub (TOCH) is a fast-paced, multidisciplinary service that supports safe, timely and effective hospital discharges across Hertfordshire. Working closely with NHS colleagues and system partners, the team manages complex discharge pathways across multiple acute hospital sites, community settings and out-of-county placements, helping people transition smoothly from hospital to community-based support.

As an Advanced Practitioner, you will support Team Managers in leading a busy social care team delivering a seven-day service. You will provide professional oversight, contribute to performance management and support service development, while using your expertise in complex hospital discharge planning, risk management and decision-making. You will work with a range of onward care options, including rehabilitation, reablement, long-term care and community support, ensuring person-centred outcomes are achieved.

We operate a seven-day hospital discharge service (8:00am–6:00pm, including weekends). You will participate in a service-wide rota, working one full weekend every eight weeks, with time off in lieu provided before and after.


Adult Disability Team West - Dacorum - Drug & Alcohol specialism

Apsley, Hemel Hempstead
18.5 hours or 37 hours per week | Permanent

Dacorum is a busy, supportive team providing support to adults with a learning disability, autism or physical disability.

This unique role combines the advanced practitioner duties with a drug and alcohol lead specialism. You will take the practice lead in drug and alcohol work with supporting and advising workers across West Adult Disability Service, case hold the most complex, be the operational lead for Making Every Adult Matter and for policy and commissioning developments”. Experience of working with adults or their carers with substance misuse would be an added advantage.


Older Peoples Team West - Prevention of Admissions Team
Apsley, Hemel Hempstead

Older Peoples Team East - Welwyn, Hatfield & Broxbourne Locality Team
37 hours per week | Permanent

East and North Herts Older Peoples Service are looking for an Advanced Practitioner who has a passion to support people to remain in their own homes and promote their independence. You will need to be able to demonstrate knowledge and skills in complex case management & safeguarding practice and be committed to working in a personalised and empowering way with our service users and carers. Experience of mentoring and developing skills and best practice within a team is essential. The role would also involve supporting with referral management, leading the team & networking with our community and hospital based social care teams to build positive relationships to promote and deliver on our principles.


Older Peoples Team East - Mental Health Inpatient Unit
Stevenage
37 hours per week | Fixed Term to Aug 2027

This role sits within a mental health inpatient team supporting people with dementia across East and North Hertfordshire. The Advanced Practitioner ensures individuals receive holistic, person-centred assessments in line with the Care Act 2014, helping to plan safe and timely discharges. The position is based in Stevenage but involves regular travel to inpatient units and participation in multidisciplinary meetings.

Key duties include completing social care assessments, contributing to care and discharge planning with clinical teams, advocating for patients’ rights and preferences, and working closely with families and carers to ensure continuity of care. The role also involves maintaining accurate records, supporting safeguarding processes, and assessing risk to promote safe outcomes.


About you

You’ll be a qualified social worker with a minimum of 2 years post qualifying experience.

In addition, you will also need the following: Actively undertakes a specialist role i.e. Moving and Handling Practitioner, Practice Educator, Approved Mental Health Practitioner, Best Interest Assessor - or agreement to undertake a specialist role within the first year of the role.

You will have extensive knowledge and experience in the field of adult social care, with an emphasis on safeguarding practice and related legislation. You will have the ability to manage competing service priorities and a sound grasp of current legislation, personalisation agenda and meeting the business objectives of the department. Demonstrating management potential, you will have excellent all-round communication and negotiation skills working with the Statutory, Voluntary and Independent sector partners.

You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support. You will utilise your knowledge of the Care Act 2014, Mental Capacity Act 2005, and other social care legislation, to manage the most complex cases. You will use of your advanced knowledge and experience to support other members of the team and the management team to ensure good practice. You will develop close working relationships with hospital staff and care providers to ensure effective care planning and balance the use of resources.
