About the role

This role is focused on responding to the needs and wishes of individuals, supporting them to lead full and meaningful lives through person-centred approaches. You will help people build relationships, connect with their local communities, and take part in activities and networks that promote inclusion, independence and participation.

You will also contribute to the planning and development of services, ensuring that people who attend the service are included and supported to take part in decisions that affect them. The role requires working within relevant departmental policies, procedures and guidance, including data protection, confidentiality, information sharing and safeguarding procedures.

About you

We are looking for someone who is compassionate, proactive and committed to helping people achieve the outcomes that matter most to them. You will take a personalised approach to care and support, promoting independence, choice, control and wellbeing.

In this role, you will need to work creatively and flexibly, building positive relationships with individuals, families, colleagues, professionals and wider community networks. You will be confident in supporting people to understand and manage risk, while ensuring their safety and dignity remain central to the support provided.

Your office base may be subject to change as we are currently transitioning between temporary and permanent office locations due to the closure of County Hall. If this is applicable to you, you will be notified, but no reimbursement will apply in respect of travel.
