Lincolnshire County Council has an exciting opportunity within our West Adult Community Wellbeing Team for an Advanced Practitioner – West Review Team.

About the Role

Working alongside the Team Manager, you will play a key role in supporting high-quality, strengths-based and person-centred practice, helping to embed innovative ways of working and driving continuous improvement across the service.

As an experienced Social Worker, Occupational Therapist, or Nurse, you will work with autonomy, managing some of the most complex cases referred to Adult Care and Community Wellbeing. You will also coach, mentor, and support colleagues, contributing to the development of a skilled, reflective, and confident workforce.

As an Advanced Practitioner within Community Wellbeing, you will be responsible for providing expert professional leadership while maintaining a varied and complex caseload.

You will be a role model for excellent practice, providing advice, guidance, and support to colleagues, and helping to ensure adults receive the right support at the right time.

This is a pivotal role where you will combine advanced practice, leadership, safeguarding expertise, and partnership working to influence outcomes for individuals and the wider service.

You must have the ability to travel across Lincolnshire and occasionally beyond county boundaries to meet the needs of the service.

You will:

• Manage a varied and complex caseload, undertaking assessments, support planning, reviews, and risk management.
• Lead and support colleagues in delivering strengths-based, personalised, and outcome-focused practice.
• Provide mentoring, coaching, and reflective supervision to team members.
• Support less experienced practitioners through co-working, guidance, and case consultation.
• Take a lead role in complex safeguarding cases, ensuring legislation, policies, and procedures are appropriately applied.
• Promote person-centred approaches that maximise independence, wellbeing, and community connections.
• Contribute to service development, quality assurance activities, and continuous improvement initiatives.
• Act as a Practice Educator, mentor Newly Qualified Social Workers, and support student placements.
• Deliver workshops, group supervision, and learning opportunities to develop practice across Adult Care.
• Build strong working relationships with health partners, voluntary organisations, communities, and other agencies to deliver integrated support.
• Deputise for the Team Manager where required.

You will have the opportunity to shape practice across the team while supporting adults to achieve the outcomes that matter most to them.

About You

You are an experienced and skilled practitioner who is passionate about delivering excellent adult social care and developing others.

You bring expertise in complex case management, safeguarding, assessment, and strengths-based practice, alongside a commitment to supporting colleagues and promoting high standards across the service.

We're looking for someone who:

• Holds a recognised qualification in Social Work, Occupational Therapy, or Nursing.
• Is registered with Social Work England, HCPC, or NMC.
• Has significant post-qualification experience working with adults with care and support needs.
• Demonstrates excellent knowledge of relevant adult social care legislation and statutory frameworks, including the Care Act.
• Has experience of managing complex and high-risk situations and applying sound professional judgement.
• Can coach, mentor, and support colleagues to develop their skills and confidence.
• Leads by example and promotes strengths-based, person-centred approaches in all areas of practice.
• Demonstrates strong analytical, assessment, report-writing, and decision-making skills.
• Is emotionally resilient, adaptable, and solution-focused.
• Works effectively in partnership with individuals, families, carers, communities, and professional networks.
• Shows professional curiosity and a commitment to ongoing learning and development.

About the Team

Our Community Wellbeing Teams work with adults across Lincolnshire who have care and support needs, helping them to maintain independence, improve wellbeing, and access the support available within their communities.

We are a welcoming, supportive, and collaborative service made up of Social Workers, Occupational Therapists, Social Care practitioners, and support staff who share a passion for delivering excellent outcomes.

You will be given a nominated contractual work base and you will be required to commute to this work base when necessary.
