Occupational Therapist

Occupational Therapist

Healthcare
£32,073 - £39,043 per year
Role Introduction

Leeds Community Healthcare is recruiting Band 5 Occupational Therapist to join either the Active Recovery Service or Neighbourhood Therapy Team. Both services deliver high-quality, person-centred rehabilitation, helping adults regain independence and remain safely in their own homes and communities.

As an Occupational Therapist, you will support adults following illness, injury, hospital admission, functional decline or changes in health and wellbeing. You will complete comprehensive assessments, develop personalised rehabilitation plans and deliver evidence-based interventions that promote recovery, independence and quality of life.

The Active Recovery Service is an innovative partnership between Leeds Community Healthcare and Leeds City Council, providing integrated intermediate care. The service delivers short-term, goal-focused rehabilitation for people recovering from trauma, orthopaedic surgery, neurological conditions, frailty, deconditioning and long-term health conditions, using a strengths-based approach to reduce ongoing care needs.

Neighbourhood Therapy Teams provide community-based rehabilitation across Leeds, supporting planned and urgent care pathways. Therapists work with individuals experiencing falls, frailty, mobility difficulties, neurological conditions and complex health and social care needs, helping to prevent hospital admission and support recovery at home.

You will manage a varied caseload, undertake holistic assessments and deliver interventions including rehabilitation, falls prevention, hospital discharge support, environmental assessments and equipment provision. Working closely with multidisciplinary colleagues, you will contribute to coordinated care planning and positive patient outcomes.

In this role, you'll have the opportunity to make a real difference in your patients’ lives while developing a broad range of clinical skills. You will work closely with a wider multidisciplinary team, enhancing your knowledge and practice through regular clinical supervision, peer support, and continuing professional development.

While you will be fully supported by senior therapists, you will also take responsibility for managing your own caseload, delivering holistic rehabilitation or appropriately delegating care.

The role also includes supervision of students, participation in service improvement and quality initiatives, and access to ongoing professional development, clinical supervision and opportunities to help shape future community rehabilitation services in Leeds.

This role is part of a 7-day service, with working hours between 8am and 6pm. Weekend and unsocial hours attract enhanced pay. We are committed to supporting a positive work-life balance through our flexible working policy. You will be aligned to either a North or West Leeds hub, with the understanding that this is a city-wide service and occasional cross-cover may be required. We will always consider individual caring responsibilities when planning service needs and rotas.

Please note that access to a car for work purposes is essential for this role.

For questions about the role, please contact Devinya Chizengeni devinya.chizengeni@nhs.net

Please review the attached Job Description and Person Specification for full details of the role.
