Role Introduction

Leeds Community Healthcare NHS Trust is delighted to offer an exciting opportunity for a Band 5 Pharmacy Technician to join the innovative and supportive Ageing Well Team team at Woodsley and Holt Park Primary Care Network (PCN). Covering a large and diverse population of over 115,000 patients across ten GP practices in North Leeds, the PCN plays a vital role in delivering high-quality, personalised healthcare in the community.

As a valued member of our multidisciplinary team, you will contribute to the safe, effective, and efficient delivery of medicines related care across a number of practices. You will work closely with Occupational Therapists, Community Matron, Physiotherapist, pharmacists, GPs, and other PCN teams, community pharmacies, community and secondary care teams. Carrying out home visits you will work to ensure optimal patient outcomes through supporting medication management including medication adherence, monitoring ability to manage medication, consideration of strategies and regular reviews at home.

As a Pharmacy Technician working within the Woodsley and Holt Park PCN, you will deliver high-quality medicines-related support across ten general practices, contributing to the safe and effective care of a diverse patient population. You will work under the direction of the Ageing Well Team with clinical supervision from the PCN Senior Pharmacy Technician and Senior Clinical Pharmacist to support clinical medication reviews, repeat prescription management, patient education, and adherence to best practice prescribing standards.

The focus of your role will be visiting patients in their own homes providing support with medication adherence, optimisation and strategies to manage medication, medication queries and referring to the clinical pharmacists as appropriate. You will also identify patients for structured medication reviews, and support the implementation of local and national prescribing guidance. You will contribute toward QOF (Quality and Outcomes Framework), proactive care initiatives, audits and patient engagement, ensuring that your work reflects a strong person-centered and evidence-based approach. This includes a combination of patient contact and non-patient contact work.

You’ll need to be self-motivated, organised, and confident working independently as well as part of a broader clinical team. You’ll play a vital part in ensuring that patients receive the right medication, at the right dose, and at the right time.

This is a very varied role that is constantly evolving in line with the diverse needs of the needs of our patients. Some daily duties can include:

- Home visits to support and monitor medication management

- Medication syncronisation

- Implementation and support with repeat dispensing

- Supporting with Long Term Condition Reviews

- Medication compliance reviews

- Patient education around medication regimes and lifestyle

- Medication safety audits

- Medicine optimisation and cost effective prescribing

- Implementation and support with enhanced services

- Supporting with prescription queries

- Contributing to developing protocols
