Our Team:
You will join our SEND Service within the Children's Services Directorate. The team deliver a variety of services across Halton, ensuring that children and young people with Special Education Needs and Disabilities are supported through to adulthood. You will report to the SEND Manager and be responsible for a caseload of children and young people.
Hours: 37 hours per week, Monday to Friday
Your Contributions:
As an Assessment Coordinator, you will deliver and coordinate Education, Health and Care Plans (EHCPs) for a caseload of child/young people, acting as the key point of contact for families and practitioners, and leading on the ongoing reviews of EHCPs for the child/young person and family.
You will lead on annual reviews of EHC Plans and deal with any issues that arise, such as the need for additional resources, funding or support (including from other services within and outside of the local authority) and provision/placement changes. You will ensure that identified funding is in place and contribute to assurance of quality and value for money.
You will work in partnership with the child/young person and their parents/carers to help them to plan towards adult life and promote independence, choice and control over all aspects of their life. This will include having clear outcomes in the EHC Plan that addresses the young person’s aspirations around employment and independent living.
