Our Team:
The service provides short-term Reablement and support to people within the Halton area. Our role is to ensure that we provide an effective assessment system that underpins service users’ care and support plans in order to support and maximise their independence; to empower them to improve their ability to live at home or become as independent as possible within their limitations.
Hours: 30 hours.Shifts will include a 4 week rolling rota involving both early and late shifts, as well as every other weekend.
Your Contributions:
We are looking for caring, hardworking, compassionate and innovative individuals who can ensure that care, support and Reablement is provided to a high standard and in accordance with care and support plans.
The successful candidate will have existing knowledge and skills in care, holding an NVQ L2 or equivalent in a care related discipline, with the drive to complete an NVQ L3. Working within a multidisciplinary team, you will develop and maintain effective working relationships with a range of professionals including Nurses, Occupational Therapists, Physiotherapists, Dieticians and Social Workers. You will possess excellent interpersonal skills and have the ability to effectively communicate with and listen to others. You will also be competent in accurately maintain clear and concise written records and producing reports.
