Our Team:
St Luke’s provides high-quality residential and nursing care for older adults, including those living with dementia and complex health needs. The home supports around 60 residents across multiple units, offering a comfortable and welcoming environment with well-equipped living spaces, dining areas, and gardens. With a strong focus on dignity, person-centred care, and community, St Luke’s is supported by a dedicated team committed to delivering compassionate and responsive care to all residents.

Hours: 24.75

Working pattern will be based on a 4 week rolling rota; you must be willing to work flexibly to meet the needs of the service.

Your Contributions:

We are currently seeking experienced Care and Support Workers to join our team. This is a rewarding opportunity to make a positive difference in people's lives by providing high-quality, person-centred support that promotes independence, wellbeing, and choice. You will work as part of a dedicated, compassionate team who are committed to delivering excellent care in a supportive and inclusive environment.

Some of the main responsibilities of the role include:

• Delivering person-centred care that promotes dignity, privacy, independence, and wellbeing, following individual care plans and providing physical, emotional, and practical support

• Assisting residents with personal care needs, including washing, dressing, grooming, continence care, and safe manual handling

• Supporting healthcare requirements by prompting, assisting with, and administering medication, carrying out basic health monitoring, and undertaking simple wound care as required

• Maintaining accurate records, monitoring and reviewing individuals’ needs, and contribute to care planning, assessments, and review meetings

• Encouraging access to community, leisure, educational, and employment opportunities while working effectively with colleagues, families, and other stakeholders to deliver high-quality support
