Our Team:
Halton Borough Council is looking to employ a Community Care Worker, to work within Halton’s Hospital Discharge Team, based at Warrington Hospital.
The Hospital Discharge Team is and an Adult Social Work Discharge Team responsible for completing timely strengths-based assessments, to facilitate hospital discharge for adults over the age of 18 years. The focus of the role will be working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties, challenging discrimination whilst promoting independence.
Hours: 37 per week, Monday to Friday
Your Contributions:
We are looking for an enthusiastic, motivated and proactive person to join our team, to provide assessments for adults who are identified as having social care needs and require support on discharge from hospital.
As a Community Care Worker you will undertake social care assessments, mental capacity assessments and work closely with health colleagues and provider agencies.
Applicants should have experience of working in a social care setting. The ability to develop supportive relationships, promoting independence and well-being are key for this role. You will be highly motivated and have experience of working as part of a team. The successful applicant will need to have good interpersonal and communication skills and be able to work well with colleagues and partner agencies, be flexible in their approach and be willing to continually improve their own practice and service provision.
