Your next role

The Bridge to Adulthood is based within Adult Social Care and is responsible for supporting young people between the ages of 14 years and 25 years to transition from children services to adult services. We work closely with the young person, their family, children services, health, schools and education to compete Care Act assessments and support plans prior to their 18th Birthday. We attend meetings to gather information to plan for the young person’s future and to provide services from the age of 18 years. We look at providing support to enable young people to become as independent as possible in all areas of their life, which can include employment, social activities, personal care and independent living. As a link worker, you will be supporting young people on a one-to-one basis to achieve positive outcomes in all of these areas alongside the allocated social worker or assistant care manager. You will also be part of the planning live events run by the transitions team.

The Bridge to Adulthood team is a supportive team consisting of social workers, social care practitioners and link workers. The link worker role also includes being part of the duty rota working alongside a duty manager.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).

The ability to travel throughout the Borough will be required and access to a car or using public transport is essential as visits in the community will be frequent.

The team operates a hybrid working model with all practitioners expected to be in the office for a minimum of two days per week.

You will be subject to an enhanced Disclosure and Barring Service Check.
