What You’ll Be Doing
In this role, you'll be part of a multi-disciplinary team supporting vulnerable Camden residents and their families, using your expertise to provide holistic, strength-based, and recovery-focused care. You'll build long-term relationships to help individuals maintain good health and improve their lives. Key responsibilities include responding to crises, recognising signs of harm, and preventing unnecessary hospital admissions. You'll also contribute to shared learning, development, and practice improvement within the Local Authority, with a focus on embedding strengths-based approaches.
It is important you have confidence and enthusiasm, as you will be speaking to members of the public and professionals at various levels, both internally and externally.
This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS.
All About You
The ideal candidate will be familiar working under the Care Act 2024 and bring with them an understanding, preferably with working experience, within a Social care setting or aligned organisation.
You will play a key role in the smooth running of the service by handling public calls, providing screening, information, advice, and escalating when needed. You'll conduct assessments and reviews, working creatively with customers to explore support options that promote independence. Additionally, you'll consider reablement needs from physical, functional, sensory, and emotional perspectives, and assist with housing allocations and resettlement as required.
