As a Band 6 Acute Liaison Nurse you will work within the Health Liaison Team supporting people with learning disabilities when they are admitted to a general hospital.</p><h4>About the role</h4><p>You will be working within the general hospital to offer learning disability expertise to reduce the risks of poorer outcomes for people with learning disabilities admitted into hospital for emergency or planned admissions.</p><p>Your skills in recommending the right approach for the individual will help your clinical colleagues and paid/family carers to get the best outcome for the person. You will be working closely with the Safeguarding Teams and Senior colleagues to develop and progress the Hospital Improvement Standards embedded into the Learning Disability Action Plan.</p><p>You will be working with Junior colleagues in the team and provide leadership and clinical supervision.</p><h4>About you</h4><p><strong>Essential criteria:</strong></p><ul>\n<li>Qualified Registered Nurse (at least 2 years’ experience post registration)</li>\n<li>Some Learning disability experience</li>\n<li>Ability to travel to alternative sites within reasonable timescales</li>\n<li>Multi-disciplinary Team working experience</li>\n<li>Able to cite evidence-based practice</li>\n</ul><p><strong>Desirable criteria:</strong></p><ul>\n<li>Car driver, with access to a car</li>\n<li>Experience as a nurse supervisor</li>\n<li>Community and hospital-based health care experience</li>\n<li>Knowledge of social model of disability</li>\n<li>Teaching skills</li>\n</ul><p><em>We welcome applications from candidates who currently reside in the UK with established proof of right to work documentation.
