This pivotal role acts as the first point of contact for emotional health referrals, ensuring that children and families receive the right support at the right time. You will be responsible for logging and progressing referrals, gathering key information from families and professionals, and coordinating multi-agency input to inform next steps. You’ll also organise weekly triage meetings, monitor case progress, and maintain accurate records.
