About the Teams

Integrated Discharge Team
Barnet & Chase Farm Hospital
37 hours per week | Permanent

You will work within a busy social care team at Barnet Hospital, carrying out assessments to support safe and timely discharges. You will collaborate closely with the hospital Discharge Team and care providers to deliver effective, person-centred care in line with Hertfordshire’s Connected Lives principles.This fast-paced role requires strong organisation, the ability to manage competing priorities, and confidence working with complex and vulnerable individuals. Using sound judgement and a solution-focused approach, you will help patients return home quickly with appropriate support, promoting independence and wellbeing.

In this post you will be required to work 7 days 1 in every 8 weeks.


Integrated Discharge Team - Watford General Hospital
37 hours per week | Permanent

The Integrated Discharge Team at Watford General Hospital support a 7 day discharge service. This is a 37 hour a week position, and 1 in every 8 weekends you would be expected to work a Saturday and Sunday.

Our work is fast paced as we focus on supporting people to leave hospital as quickly and safely as possible, with a person-centred approach, with the majority of the work being through Discharge to Assess.

The usual hours are 9am – 5pm and you would need to be able to work in Watford General Hospital every. Due to the nature of the hospital work, there is no remote or hybrid working in this role.

37 hours per week | Fixed Term

You will work as part of a busy Transfer of Care Team, completing social care assessments to ensure that people are discharged in a safe and timely manner. You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support. You will develop close working relationships with hospital staff and care providers to ensure effective care planning.


About you

Essential criteria

  • The team is community based and you must have a full UK driving licence and regular access to a vehicle for working purposes
  • Experience of working in social care
