Are you looking for a rewarding career where you can make a difference to the lives of adults in Leicestershire? Are you passionate about promoting independence?

About the Role

As a Community Support Worker within the Domiciliary Review Service, you will undertake direct work with individuals, families and carers to connect and listen. You will undertake a Review of the support in place to meet their eligible needs. This will involve looking together at their current Support Plan ensuring it is achieving the identified outcomes. Where ongoing eligibility is confirmed following the Review, you will re-visit the Support Plan ensuring that it utilises available community resources and connecting them to widen opportunities. This may include referrals to other agencies and dealing directly with a variety of professionals and organisations. Where necessary, you will identify and report potential safeguarding, consider mental capacity, and other high-risk situations where a person’s safety or liberty may be at risk. You will undertake visits to people in their own homes, or other venues as directed by the individual. We also work proactively with carers ensuring any impact is identified and addressed at the point of review.

We have vacancies in the following teams:

  • Domiciliary Review
