Working across GP practices, NHS 111, out-of-hours services, and community settings, our skilled clinicians, nursing staff, and support colleagues collaborate to deliver high-quality, patient-centred care. As a Clinical Pharmacist you will:

  • Deliver expert clinical medicines management, including structured medication reviews, medicines reconciliation, and support for patients with complex polypharmacy.
  • Work as part of a multidisciplinary team across the practice and Primary Care Network to manage long-term conditions, improve health outcomes, and reduce health inequalities.
  • Lead on safe, effective prescribing systems, including repeat prescription processes and medicines optimisation across transitions of care.
  • Contribute to quality improvement, clinical audit, and the Quality and Outcomes Framework, driving service efficiency and high standards of patient care.
  • Provide specialist pharmaceutical advice, education, and independent prescribing support, promoting best practice and alignment with local and national healthcare priorities.
