As a Clinical Pharmacist you will:

Deliver expert clinical medicines management, including structured medication reviews, medicines reconciliation, and support for patients with complex polypharmacy.
Work as part of a multidisciplinary team across the practice and Primary Care Network to manage long-term conditions, improve health outcomes, and reduce health inequalities.
Lead on safe, effective prescribing systems, including repeat prescription processes and medicines optimisation across transitions of care.
Contribute to quality improvement, clinical audit, and the Quality and Outcomes Framework, driving service efficiency and high standards of patient care.
Provide specialist pharmaceutical advice, education, and independent prescribing support, promoting best practice and alignment with local and national healthcare priorities.
