Job Introduction

The Role:

If you are a qualified social worker with experience of working within the field learning disabilities and/or mental health and you are looking for a role where your professional judgement, skills and knowledge is trusted and valued then this could be the opportunity for you.

You will work within a small project team working in partnership with individuals and their carers to complete Care Act assessments, develop strengths based support plans and review outcomes that genuinely improve peoples lives. The Pathway Planning Team work closely with providers to ensure that services continue to be effective in meeting the needs of individuals. You will bring confident decision making, analytical skills and attention to detail along with effective communication and the ability to work collaboatively with multiple agencies.

This is a role where you will really develop your practice, your analytical skills and attention to detail whilst having the opportunity to work with a variety of different services.

You will be a small team countywide team where you will recieve regular supervision and reflective practice sessions. We offer learning and development opportunities and a hybrid way of working that balances time in the office with working from home.
