The Team dedicated to ensuring people are receiving the support and care they need to maintain their independence and wellbeing, we are responsible for Sourcing Packages of Care and support from the market place for people who may be funding services themselves or eligible for Health or Social Care funding, the services include but are not limited to Care Homes, Domiciliary Care, Supported living, Extra Care, Reablement, Day Services. <br><br><b>You will</b>;<br> <br> <ul> <li>Work with the person and their representatives to identify and source suitable provisions under a number of contracts to ensure they are appropriately supported,</li> <li>Liaise with care providers, social workers, and legal professionals to advocate for the best interests of the individuals.</li> <li>Negotiate placement and package costs with the providers</li> <li>Maintain accurate and timely records.</li> <li>Cultivate and sustain positive relationships with both internal and external stakeholders.</li> </ul> <br><br><strong>Key Responsibilities:</strong><ul> <li>Managing own case load and meeting strict time frames</li> <li>Ensuing all services are meeting their contractual obligations</li> <li>Prioritise cases in emergency situations and ensure quality and contractual requirements are met.</li> </ul> <br><br><strong>Work Style</strong><br> <br>At North Somerset Council, we offer a hybrid working environment based on four distinct work styles.
