Outreach Worker
We are currently seeking an Outreach Worker to join our dedicated team.&nbsp; We recognise that the transition from the Approved Premises to community living can be a difficult time for our residents.&nbsp; As Outreach Worker, you will support residents remaining in the Reading area, both prior to their move on and for up to six months post move on.&nbsp; The aim is to support them living independently, with practical matters and signposting to additional support services within the community.&nbsp;
This is a part time post, working 20 hours a week.&nbsp; The proposed working pattern is Monday – Friday, 1pm – 5pm, but we can consider alternative working arrangements.&nbsp; &nbsp;

Key Responsibilities:

<li>Identify residents that will be remaining in Reading at the earliest opportunity and work with them to prepare for move on, completing needs assessments and support plans.</li>

We are looking for Deputy Team Manager Occupational Therapist for Brent’s Reablement Team which is&nbsp;an integrated service combining the fast-paced response to Pathway 1 Home First discharges and the delivery of reablement and social care services in a short term response team set up. The team’s ethos is about promoting and enabling independence and we have good working relationships with our partners and colleagues in the hubs and intermediate care.&nbsp;
This is an excellent opportunity for an experienced Occupational Therapist, who have experience of working with adults with physical disabilities and older people to take an active role with day to day running of the Team in providing specialist advice, guidance, support and supervision to qualified and non-qualified social care and OT staff working within the team. About the role
Our residents in this service have experienced long term street homelessness and have faced significant challenges including with their mental health, substance use, and alcohol dependency, and struggled to sustain engagement with support services. That's why they're here, to make a change and gain the support they need to enable greater independence and recovery.
In this role, as a Support Mentor, you will work within a 24 hour service which provided person centred support to our residents. Some key responsibilities include:
<ul>
<li>Ensuring the safety and wellbeing of residents and participants.</li>
<li>Be a present member of the team on the night shift, completing handovers for day colleagues for a seamless service.</li>
<li>Building trusting professional relationships with residents, helping them to achieve their goals and aspirations which could be through tailored one to one sessions, group sessions, in and out of service activities, and general wellbeing check ins.</li>
<li>Focusing on empowering our residents to take control over their lives and meet their personal goals.&nbsp;</li>

About you
