 Your role will include:

<li>Providing a timely transition for all Care Act eligible 14- to 18-year-olds into adulthood; and ensuring all adults with a learning disability and/or autism are receiving the appropriate ASC support under care act eligibility criteria.</li>
<li>Promoting as much independence, whatever that looks like to the young/older adults, as possible to allow for successful and achievable outcomes.</li>
<li>Embedding an enablement focus. You will support individuals to remain in their own homes and maximise their independence.</li>
<li>Working with all families to ensure they understand the role and remit of ASC for eligible LDN adults.</li>
<li>Developing close and effective relationships with providers, community and voluntary services, children’s services, schools and colleges.</li>

 About the role
We currently have a vacancy for an experienced social work qualified Assistant Team Manager to join our Supporting Families Team, part of the Family Help &amp; Safeguarding service. We firmly believe that relationship-based social work makes a real difference to the lives of children, young people and their families, and our Supporting Families Team focus is on delivering outstanding support to families.
 Our Supporting Families Team currently provides a range of interventions including supervised contact for Children Looked After; specialist support for parent/carers with substance misuse difficulties via a Consultant Practitioner; providing direct work and parenting interventions for those children subject to CP Plans, and offering group work support for parents/carers who are survivors of domestic abuse. There is now an exciting development within the Team to introduce a small group of Lead Practitioners to case hold Child in Need (S.17, Children Act 1989) cases in line with the social work reforms, with the support of a social work qualified and experienced Assistant Team Manager.
As this Assistant Team Manager, you will be responsible for leading and supervising a small team of 7 Lead Practitioners who will each case-hold a number of Child in Need cases, taking new cases from our Together for Families Teams following the completion of Single Assessment. You will provide passionate and inspiring leadership to your Team, providing regular high-quality supervision and support - including chairing meetings and conducting joint visits with the Lead Practitioner as required. You will ensure that the child’s lived experience and the promotion of their welfare is at the very heart of everything that is done. Location:&nbsp;Face to face service based in Royal Borough of Kensington and Chelsea (RBKC), the service is a short walk from Ladbroke Grove (Circle and Hammersmith) station. Unfortunately, this service does not have step free access.
Salary:&nbsp;£27,000
Shift Pattern:&nbsp;37.5 hours per week Monday to Friday on a rota basis between 09:00 - 17:00 and 13:00 - 21:00. You may be required to work bank holidays and evenings as required.
About the role
We are seeking a compassionate and dedicated Support Mentor to join our team based in RBKC to support our residents who have multiple and complex needs which could include mental health, substance use, homelessness, and/or offending backgrounds. You will support in providing our residents with the support needed to help them overcome their personal barriers, and achieve their goals in a person centred approach. You will work within a residential setting, creating a safe and comfortable space which our residents can call their homes during their time within the service. The role includes:

<li>Supporting residents with daily tasks</li>
<li>Holding a caseload, conducting key work sessions which are personable to their needs</li>
<li>Carry out regular welfare checks, safeguarding checks, and general living checks</li>

About you

You will be able to build rapport and trusting relations in a professional setting, and able to build positive outcomes. You will have an adaptable and flexible approach to helping others, with an understanding that each individual has different levels and types of support needs.&nbsp;

<li>Experience of working with people of complex backgrounds and/or a good understanding of the sector, particularly around multiple and complex needs</li>

About you
This role is ideal for someone who has a real passion towards helping others to achieve greater independence. We're looking for someone with a genuine passion and felt purpose to help people, challenge stigma, and make a real difference to people’s lives! You will have:
<ul>
<li>Understanding of the housing support needs that people with multiple, and complex needs face</li>
<li>Experience of working with people of complex backgrounds and/or a good understanding of the sector</li>
<li>Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs</li>
<li>Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents</li>
<li>Understanding and/or practical knowledge of the social and societal marginalisation that can be attached to people with complex needs</li>
