Job Title:&nbsp;Housing Support Worker
Location:&nbsp;Non residential service which is based in Brent (Close to the Civic Centre), you will provide support based in the main office and in the wider community, including homes
Salary:&nbsp;£27,000
Shift Pattern:&nbsp;37.5 hours per week Monday to Friday on a rota which can range between 08:00 - 16:00, 09:00 - 17:00 and 10:00 - 18:00. You may be required to work outside these hours dependant on service and participant requirements.&nbsp;
About the role
This is an exciting opportunity to support individuals with multiple, complex needs on their journey towards recovery, reablement, and independence, all within a compassionate, trauma-informed environment.
In this role, you'll be at the &nbsp;heart of delivering high-quality, person-centered support to vulnerable adults who have been or are at risk of homelessness. They are identified as having low/medium support needs. You will create tailored support plans to provide practical interventions, to empower our participants to overcome personal challenges, develop life skills, and achieve their personal goals. You will also work closely in partnership with various organisations to support the different needs of our participants, which may include signposting. Social Care Officer £37,068 - £38,058 per annum About the Role An exciting opportunity has arisen for a newly created Social Care Officer post within the Mental Health Adult Social Care Access Team (MHASCAT). MHASCAT is the front door service for all new referrals into mental health social services which sits within the Primary Care Networks (PCN) space in conjunction with our health colleagues. We are looking for a Social Care Officer who wants to develop their skills with case work under the Care Act 2014, undertaking assessments of need, completing support plans and working with residents and carers to maximize independence and improve well-being, ascertaining the level of need and signposting as appropriate. Other duties include but are not limited to, daily screening of all new referrals that come through our Duty system. This includes referrals from health (including GP’s and hospitals), other teams within LBN such as Children’s Services and housing, out of borough cases that may require thorough background research, and self-referrals from residents and their carers and families. You will also take phone calls from the general public, giving advice to residents and families about what support is available to them within LBN social services as well as have knowledge of charities and other partner organisations within Newham. As a Social Care Officer, you will need to be able to make judgement calls with your line manager on the appropriateness of the referral, the type of support that you feel the resident requires, and signpost accordingly. Most of the role will be over the telephone and virtually, however you may be asked to undertake emergency home visits if and when required. There is a strong commitment to supporting professional development, to improve services and outcomes for vulnerable people. About the role
We're seeking a Deputy Service Manager to join our Independent Approved Premises (IAP), commissioned by Criminal Justice Service (IAP) which works with high risk offenders who have left prison. Penrose Drive is a community setting, rather than custodial where residents can go out in the community if they do not have restrictions on their license. Security is therefore flexible in both design and operation. The team work in a multi disciplinary approach, working alongside probation services, job centres, mental health teams, and other community partners.&nbsp;
You will lead the team by providing line management and leadership support to enable and empower the team to deliver high quality support to our residents. You will ensure a quality service is delivered in line with our contractual requirements, and will encourage the creation of a psychologically informed environment, which provides person centred support. The role includes:
