In Shared Lives, a person lives or stays with a Shared Lives carer within the local community. The Shared Lives carer provides the care and support in their own family home and as part of their own family and community network. The person would become part of the carer’s extended family and be supported by the Shared Lives carer to live the life they choose.

Brent shared lives scheme is a CQC (Care Quality Commission) registered service offering the opportunity for vulnerable adults to live in the community.&nbsp; The adult is “matched” with a carer who is recruited, assessed, trained, and supported by scheme officers.&nbsp;&nbsp;

The successful applicant will provide specialist advice, guidance, and supervision to OT Assistants, ensuring high standards of professional practice within the team. The Person

We’re looking for a Night Recovery Worker to join our 24/7 supported accommodation service based in Enfield. You will work in a vibrant care and rehabilitation service, offering 21 beds to adults with complex mental health needs. You will join a team which consists of a Service Manager, Deputy Service Manager, a day team of 10, and a night team of 4. You will be a present member of the team on the night shift, with a minimum of 2 people working per shift. We are here to promote independence and give our residents the tools needed to fulfil their lives in the community independently.
In this role, you will play a key role in supporting our residents on their journey to recovery, reablement, and rehabilitation, you’ll support individuals transitioning from complex care or acute mental health services to a structured rehabilitation setting. Working closely with various teams, you’ll help provide tailored, person-centred and trauma informed support for residents to overcome personal challenges, and meet their desired goals/outcomes and support medication compliance, making a real difference every day.

Co-located with health colleagues at HUC, the team attend to residents of Hertfordshire over the age of 18 at risk of avoidable hospital admission.<h4>About the role</h4>EIV Practitioners are interdisciplinary professionals with a focus on crisis management, social care assessment, functional assessment and equipment provision within the context of urgent response. Working in close partnership with Urgent Care Practitioners (UCPs) from HUC to form a 2 person team, you will collaborate with your health colleague to devise care plans and work effectively across teams and agencies within health, social care and community services to manage risk, support onward care and prevent further crisis episodes.
