We are seeking a dedicated and compassionate Recovery Worker to join our team at, based in West Ealing. We support adults experiencing homelessness, isolation, mental health challenges and addiction. We provide support and appropriate interventions that will enable successful resettlements, and deliver to achieve safer, integrated communities for our stakeholders, You will provide pathways, support, and appropriate interventions to enable successful resettlements and collaborate with internal and external partners and agencies to achieve desired outcomes. &nbsp;
We have two multi-bed premises Churchfield and Cherington, where we provide supported housing for individuals facing addiction and homelessness. Churchfield offering 24/7 temporary support for those in active addiction and Cherington supporting abstinent residents to reintegrate into the community.&nbsp;

We are seeking a Dual Diagnosis Worker who has a specialism in mental health and substance use to join our team in Croydon. You will be based within a residential service which provides specialist mental health, complex needs, and dual diagnosis support to our residents. You will deliver evidence based interventions based on holistic assessments to support personalised recovery, integration, and promotion of independent living skills. The Post
Shared Lives is an alternative to traditional types of care and support available to adults with a learning disability. In Shared Lives, a person lives, or stays with a Shared Lives carer within the local community. The Shared Lives carer provides the care and support in their own family home and as part of their own family and community network. The person would become part of the carer’s extended family and be supported by the Shared Lives carer to live the life they choose.
Hackney Shared Lives supports people who rely on the help and support of others to maximise their potential and maintain their sense of independence.
Hackney shared lives scheme is a CQC (Care Quality Commission) registered service offering the opportunity for vulnerable adults to live in the community. The adult is “matched” with a carer who is recruited, assessed, trained and supported by scheme officers.
The role of the registered manager is to support this process and ensure the compliance of the scheme within the remit of CQC. Each placement is different and this brings challenges and rewards.The registered manager is responsive to CQC, Shared Lives plus (who offer oversight of all 150 schemes nationally) and also works alongside the multidisciplinary Mental Health and Learning Disabilities teams who refer to the service.
Shared Lives Carers are self employed and supported to ensure their fees are competitive and vacancies are advertised. The team has enjoyed a period of stability, including the restructure of the scheme financially and procedurally. The scheme is ready to grow and increase the offer of community living to vulnerable people in Hackney.
The Person
Whilst we value experience, you will need to be committed to providing quality service and opportunities to the people living within Shared Lives. You will need to demonstrate your understanding of supporting people on the Autism Spectrum and Learning Disabilities. You will need to evidence leadership qualities, flexibility, enthusiasm, initiative, and resilience, for what is often a very demanding but rewarding vocation.
Your role will include being a CQC registered manager for the Shared Lives Service, supervising the scheme officers, supporting carers and vulnerable people, organising relevant training, providing reports to various stakeholders and being prepared for the possibility of a CQC inspection. The registered manager takes the lead role for the scheme in matters of safeguarding. The service is always aiming to be “outstanding” with CQC and take an innovative approach to social care and quality assurance improvements - which is a key role of the registered manager. Hackney is a culturally diverse borough and the scheme works to support and recruit potential carers from all parts of the community - building relationships with the community groups is essential. We are looking for Deputy Team Manager Occupational Therapist for Brent’s Reablement Team which is&nbsp;an integrated service combining the fast-paced response to Pathway 1 Home First discharges and the delivery of reablement and social care services in a short term response team set up. The team’s ethos is about promoting and enabling independence and we have good working relationships with our partners and colleagues in the hubs and intermediate care.&nbsp;
This is an excellent opportunity for an experienced Occupational Therapist, who have experience of working with adults with physical disabilities and older people to take an active role with day to day running of the Team in providing specialist advice, guidance, support and supervision to qualified and non-qualified social care and OT staff working within the team. Also the opportunity to develop the team services and role in delivering the Integrated Care Partnership.&nbsp;

Improved pathways with the existing community social care and intermediate care services as part of the Integrated Care Partnership with our Acute and Community NHS provider Trusts and 3rd Sector organisations
Working with Brent’s Adult Social Care brokerage team on training of new Reablement Home Care providers.&nbsp;
Staff development and training for team members as an integral role in the retention and upskilling of the workforce in Adult Social Care.&nbsp;
The successful candidates will ideally have post graduate experience in Social Care and or Intermediate Care, have experience of supervising staff and or students in the work place and of working with Home Care providers and/or Reablement providers.
