THE SERVICE;

The Woodlands is a Supported Living Service consisting of 10 individual flats. Individuals hold their own tenancy with Wyre Forest Community Housing. The service is registered with the Care Quality Commission (CQC).

We provide care and support to people in their own homes to enable them to live as independently as possible both at home, out and about in the community. The level of support required by each individual is guided by them, their Personal Assessment and support plans. The support provided ranges from assistance with personal care, cooking, shopping, attending social events, building social networks/friendships and relationships, fitness/ healthy lifestyles, appointments, budgeting and support to manage their homes, just to name a few, whilst remaining person centred allowing flexibility and choice. We are a transitional service with good working partnerships within the community. Our goal is to support tenants until they can live as independent as possible.

The service supports adults 18+ with a Learning Disability, Mental health, neurodiverse diagnosis, physical disabilities, sensory needs and emotional / psychological needs. Experience with this would be desirable but not essential.
