Case Management Systems Officer ( LiquidLogic Social Care and Early Help)

We are seeking a skilled and motivated individual to support our current Liquid Logic Analyst in delivering system support across a range of Children’s Services teams. This role offers the opportunity to work collaboratively with various departments to enhance the efficiency and effectiveness of our Liquid Logic database system and the associated processes. 
The systems officer will be office based, providing onsite support to all Children Services teams, including, but not limited to MASH, Children in Need/Child Protection, Children in Care, Business Support, Leaving Care, Joint Service for Children with Disabilities, EISS, Fostering, Special Guardianship, Data and performance team, and Placements. 
You will be expected to deliver exceptional customer service, by providing quick and efficient support, advice, and new information. To do this, you must be able to confidently communicate with the workforce, whether this be face to face, by phone or by written format. You should demonstrate solution focused thinking, coupled with a high level for attention to detail.

Social Care Officer £37,068 - £38,058 per annum About the Role An exciting opportunity has arisen for a newly created Social Care Officer post within the Mental Health Adult Social Care Access Team (MHASCAT). MHASCAT is the front door service for all new referrals into mental health social services which sits within the Primary Care Networks (PCN) space in conjunction with our health colleagues. We are looking for a Social Care Officer who wants to develop their skills with case work under the Care Act 2014, undertaking assessments of need, completing support plans and working with residents and carers to maximize independence and improve well-being, ascertaining the level of need and signposting as appropriate. Other duties include but are not limited to, daily screening of all new referrals that come through our Duty system. This includes referrals from health (including GP’s and hospitals), other teams within LBN such as Children’s Services and housing, out of borough cases that may require thorough background research, and self-referrals from residents and their carers and families. You will also take phone calls from the general public, giving advice to residents and families about what support is available to them within LBN social services as well as have knowledge of charities and other partner organisations within Newham. As a Social Care Officer, you will need to be able to make judgement calls with your line manager on the appropriateness of the referral, the type of support that you feel the resident requires, and signpost accordingly. Most of the role will be over the telephone and virtually, however you may be asked to undertake emergency home visits if and when required. There is a strong commitment to supporting professional development, to improve services and outcomes for vulnerable people. Sefton Council is seeking to recruit 2 experienced Contracts Managers working within its Adult Social Care Commissioning and Transformation Team.
The role will:

<li>Be responsible for managing and monitoring the quality and performance of commissioned contracts, service agreements and other formal arrangements between the Department and providers to ensure the quality and efficiency of services commissioned under these arrangements.</li>
<li>Take a lead role, with the Head of Service, with regards to the quality of provision and outcomes provided by commissioned providers.&nbsp; </li>
<li>Manage all end-to-end contracts processes and coordinate activities to support the availability of sustainable, high-quality services in the Borough.&nbsp;&nbsp; </li>
<li>Ensure analysis of quality concerns are maintained and reported to senior managers, as required, with regards to quality issues, safeguarding, potential market failure, or reduced capacity which could impact on the safety or availability of provision</li>
</ul>

About the Role
As a Children's Residential Practitioner (Waking Night), you will play a vital role in bringing our vision to life. We need your passion and energy to help ensure our children feel loved, safe, happy, healthy and empowered to achieve their aspirations.
In this role, you'll provide person-centred care based on individual support plans, ensuring each child's needs are met with dignity and respect. Your responsibilities will include maintaining a safe and clean home environment, supporting with meal preparation and domestic duties, administering and recording medication in line with policy, and ensuring health and safety standards are met.
You'll also contribute to care planning, goal setting, record-keeping, and provide clear handovers to colleagues. Outreach work (e.g. hospital or police station visits), attending training and meetings, and actively participating in supervision and appraisal processes are also part of the role.
Waking night shifts typically run from 21:45 to 07:45, covering a 10-hour shift. Working hours and shift patterns will be scheduled based on the operational needs of the service. We are currently recruiting for 1x 10 hour per week position, equivalent to one shift per week. There may also be opportunities to take on additional hours-for example, to cover annual leave or staff training.

<li>Employees who work at night as part of their normal working week are entitled to receive an enhancement of time and a third for hours worked between 10pm to 7am.</li>

For overtime on Sunday, general public /bank holiday, payment is made at double time. Overtime is based on the grade rate and is not enhanced by other allowances paid, e.g. night duty.</li>

Working within Turn Furlong (Kingsthorpe, Northampton), to provide a new integrated health and social care model of care for people needing short term support upon discharge from hospital.The 'Recovering Independence Bed' unit will be delivered by a range of staff, from both health and social care settings.
