The Merton Integrated Learning Disability Team provides support to adults with Learning Disability and their carers to maintain independence and wellbeing and safety.

The team work closely with a wide range of colleagues across Adult Social Care, health services, and partner agencies to deliver coordinated, person-centred support and achieve positive outcomes for adults and carers.
