Practice Development Lead 

We are currently recruiting for a Social Worker within our integrated discharge team to contribute to the delivery of highly effective children and adult services (within Adult Social Care) in an efficient manner. The service is delivered using a strength based, personalised approach.We work across both the acute and non-acute sites across Medway and out of area hospitals. Social Worker - Hospital Integrated & Community Discharge Team 

We are currently recruiting for a part-time Young Persons Tracking Officer, to support data collection activities for young people academic age 16-17 years, for NEET and UNKNOWN reporting. The main purpose of this role is to support statutory tracking activities for Medway’s young people, offer support and sign posting to appropriate provision, build positive working relationships with education and training providers to promote services and maintain a supportive social media presence within the IAG Team.
