Are you a qualified Social Worker who is passionate about helping people regain independence, return home safely and live well in their own communities? This is an exciting opportunity to join Oxfordshire County Council’s Home First Neighbourhood Team, working at the heart of hospital discharge and community recovery.
In this role, you will be part of a committed, multi-disciplinary service supporting people through the Discharge to Assess model. You will work closely with hospital colleagues, community teams, reablement providers, therapists, discharge coordinators, voluntary sector partners and families to make sure people receive the right support at the right time. Your work will directly contribute to safe, timely and effective discharges from hospital, while helping people maximise their independence and achieve the outcomes that matter most to them.


About the role


You will carry out timely, proportionate and strength-based assessments, usually within 72 hours of discharge, identifying creative and practical ways to support people to recover, build confidence and remain as independent as possible. This may include arranging reablement, equipment, adaptations, assistive technology, informal support networks and community-based solutions.
You will apply your professional judgement within the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 and relevant safeguarding frameworks, ensuring that people are supported safely, legally and respectfully. You will also maintain clear, accurate and professionally defensible records, contribute to service improvement and take part in a weekend rota, with time off in lieu and a 5% salary enhancement once weekend working commences.


What you will bring

  • A recognised Social Work qualification and registration with Social Work England.
  • Experience of strength-based assessments, reviews and person-centred support planning.
  • Confidence working in partnership with health colleagues, families, carers, providers and wider community services.
  • Sound knowledge of adult social care legislation, safeguarding, confidentiality and information sharing.
  • Strong communication, decision-making, organisational and record-keeping skills.
  • A calm, resilient and flexible approach, with the ability to prioritise effectively in a fast-paced hospital discharge environment.
  • A commitment to equality, inclusion, professional development and achieving positive outcomes for people.
