You will work peripatetically within our Community Team, which will mean that you will provide cover for the existing staff team when they are not in the workplace. Our Community Team provides sessional support enabling individuals to access activities of their choice within their local communities. The main purpose of the role would be to consider the physical, emotional, cultural and social needs of any individual receiving support - whilst making peoples' sessions enjoyable and meaningful. A person-centred approach to support for individuals will ensure that opportunities to promote their independence, maintain friendships and improve their wellbeing will be achieved. You will be required to contribute and deliver an evidence-based weight management programme to improve health and wellbeing in Medway by supporting the achievement of local and national strategies relating to Public Health.
