About the role:

Making A Difference as an Assessment and Review Worker in Later Life
We are currently looking to recruit a permanent Assessment &amp; Review Worker into the Later Life, Disability, Mental Health, DOLS and All Age Review Team. The team is based at Forster House and consists of Social Workers and Assessment and Review Workers. The team has a number of functions including managing the reviews across all adult services.
The role of the Assessment and Review Worker is to arrange and manage reviews for individuals who receive support provided by the Local Authority. This will include individuals supported within a variety of settings including in their own home, in residential or nursing care, within sheltered accommodation and supported living settings. The reviews will ensure the support provided continue to meet the individuals care needs as defined under the Care Act 2014.
This is an excellent opportunity to gain and develop experience within a Social Work setting. The team is a supportive team and the Assessment and Review Workers will work alongside Social Workers and Managers.
A good induction and monthly supervision will be provided as well as ongoing peer support.

About the role
We are seeking a dynamic and self motivated Intensive Needs Support Coach to join us on a fixed term rolling contract as part of the Rough Sleeper Initiative Project. You will provide effective practical and personal support and guidance to our residents during their stay in our accommodation to enable them to become more resilient and to live independently. The service supports residents who have a history of non-engagement and have a history of rough sleeping and complex needs. You will proactively support them to be more engaged in activities and support plans to overcome their personal barriers. You will run activities and complete safety plans, support plans, and other appropriate interventions to support our residents to achieve their goals.
About you
We are looking for someone who is dedicated and passionate to supporting those who face multiple disadvantages within society, and have a history of homelessness. You will be driven to help end the cycle, by providing personalised support to individuals for them to gain greater independence and have long term access to accommodation. You will have an understanding of the challenges they face and understand how different complex needs can vary. We're looking for someone who is resilient, a problem solver, and able to work in fast paced ever-changing environments.&nbsp;

<li>Experience in working directly with people who have experienced homelessness</li>
<li>Experience with supporting people with multiple complex needs which can further include substance use and mental health</li>

Working in partnership with other officers, agencies, and community organisations, the Age Friendly Community Coordinator will play a pivotal role in promoting inclusivity, well-being, and active participation among older residents, in alignment with the Council’s corporate priorities.
Awareness of the needs and concerns of older people, including social isolation, health disparities, and economic barriers together with familiarity with local and national policies that impact on the lives of older people would be useful.
