Hackney’s Housing Department is changing. We are looking for an experienced, enthusiastic and dynamic Social Work Lead to be based within our new Tenancy Sustainment &amp; Community Cohesion Service and to work across Housing and Adult Social Care via a matrix management arrangement.
A recent service review highlighted the need to improve our ways of working with residents who have a range of health and social care needs and where those needs are jeopardising the sustainability of their tenancy.
We have strengthened the existing Tenancy Sustainment function to include additional specialist staff, and we see the Social Work lead as a crucial member of the new team.
Alongside the wider Tenancy Services team, you will undertake a dual role:

<li>To support the design, delivery and continuous improvement of the Tenancy Service, acting as an important bridge between Housing and social care and using your strong interpersonal skills to build knowledge, collaboration and seamless joint working across the system</li>
<li>To provide assessment and direct case management of a caseload of Hackney Council tenants, providing short to medium term crisis intervention and tenancy sustainment support to reduce risks and promote resilience and recovery</li>

In this critical new role, you will have an opportunity to set the direction of change as we modernise into a truly resident-centric service, with accountability, fairness and respect at the heart of everything we do.
You will be comfortable working alongside Housing and Social Care staff and supporting them to build their skills and knowledge to better meet the needs of more vulnerable residents. You will be involved in a range of service development projects as well as undertaking casework. This may include delivering training, developing new policies and procedures and other caseworking tools to improve how we support more vulnerable residents to live well and thrive. Key areas of work will be around supporting residents who:

<li>Are experiencing antisocial behaviour or domestic violence and abuse as either a perpetrator or a victim / survivor</li>
<li>Are experiencing a range of physical and mental health challenges that are affecting their ability to sustain their tenancies and wellbeing, including those lacking mental capacity to make decisions about their housing, people unable to return home from hospital due to their living conditions etc</li>
<li>Are currently hoarding or experiencing poor living conditions connected to their health and social care needs</li>
</ul>
You will hold a UK state registered degree in Social Work, DipSW or CQSW and be registered with Social Work England for a minimum of 3 years. You will demonstrate an excellent knowledge of the Care Act, Mental Health Act and Mental Capacity Act / DOLs and be able to demonstrate a strong knowledge of safeguarding as it pertains to both children and adults. You will also have a good working knowledge of the social housing sector and legislation including housing supply and demand issue

Shift Pattern:&nbsp;12 Month Fixed Term Contract 37.5 hours per week on a rolling rota between Monday to Sunday from 08:00 - 20:30 including a mix of longer shifts 3 to 4 days a week. You may be required to work outside these hours as per service requirements such as bank holidays and evenings.&nbsp;
<h4>About the Role</h4>
We are seeking a dedicated and compassionate Recovery Worker to join our team based in West Ealing . We support adults experiencing homelessness, isolation, mental health challenges and addiction. We provide support and appropriate interventions that will enable successful resettlements, and deliver to achieve safer, integrated communities for our stakeholders. The service has two 24/7 multi-bed premises which provide supported housing for individuals facing addiction and homelessness.&nbsp;
In this role, you will work closely with our residents to provide tailored person-centred support to enable our residents to feel empowered to overcome personal challenges, and achieve their desired goals. You will support them with re-integrating into the community through tailored support plans, interventions, and working closely with our stakeholders.

<li>Be a valued and reliable member of the team, completing handovers for night colleagues for a seamless service</li>

<li>Understanding of the different needs people with multiple and complex needs have and be aware of the social marginalisation that can be attached to people who face personal challenges</li>

Two of these teams will work exclusively with children and families whose lives are influenced by disability or SEND to ensure that we retain the expertise necessary to support these families.
