This role has a starting salary of £28,546 per annum (pro rata for part time roles), based on a 36-hour working week.

We are excited to be hiring new Reablement Practitioners to join our fantastic Reablement teams across Surrey. We have teams based in:

  • East 1 – Redhill, Reigate, Horley and Tandridge
  • East 2 – Banstead, Epsom, Mole Valley and Elmbridge
  • West 1 – Guildford, Waverley, Surrey Heath and Farnham
  • West 2 – Runnymede, Spelthorne and Woking

We have full time roles of 36 hours per week and part time roles from 32 hours per week and above.

About the Team

Our Reablement Service, within our Adults, Wellbeing and Health Partnerships Directorate, has undergone a year of transformation, creating new opportunities within our Reablement Practitioner roles. As a therapy-led service, Reablement supports individuals to regain independence following an illness or accident.

About the Role

As a Reablement Practitioner, you will work closely with individuals to help them achieve the SMART goals set out in their reablement support plan. You will carry out assessments and contribute to regular reviews, ensuring that records are accurate, timely and maintained using mobile technology.

The role requires providing respectful, person-centred support with personal care tasks, including assisting individuals with washing, dressing and other aspects of daily living including mobility, medication prompts and the safe use of equipment.

You will act as a trusted assessor, assessing the need for equipment, ordering and fitting items in line with your training, and monitoring ongoing safety. Working collaboratively is central to the role and you will liaise closely with Occupational Therapists, fellow Reablement Practitioners, families, carers and partner agencies. You will also attend daily team huddles to share updates on progress and help ensure support is well coordinated. Throughout your work, you will promote independence, dignity, wellbeing and choice, while helping to prevent hospital admissions and supporting individuals to return home safely.

We welcome applicants with or without previous care experience – full training will be provided.

Shortlisting Criteria

In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours:

  • A passion for supporting people to live independently.
  • The ability to communicate sensitively and effectively in challenging or emotional situations.
  • Confidence using mobile devices and basic IT systems.
  • The ability to plan your workload, work within timescales and adapt to changing situations.
  • Willingness to work towards relevant qualifications (e.g., Level 2/3 Health & Social Care).
  • Understanding of equality, diversity and safeguarding (or a willingness to learn).
  • Ability to meet the physical requirements of the role and the flexibility and ability to work a two-weekly rota, including evenings one weekend in two.

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. Reasonable adjustments where needed will be made for successful applicants who have a disability or long-term health condition to enable them to fulfil the requirements of the job.
