Connected Lives Gateway Service

Our Connected Lives Gateway Service is a County Wide multi-disciplinary service delivering social care solutions to all age adults. This service is the front door to Adult Social Care in Hertfordshire and aims to prevent, reduce and delay need by ensuring people can access a wide range of information, advice, and support in a timely way. Our focus is on prevention and enablement, connecting people to their communities and delivering interventions that lead to improved outcomes.

Older Peoples Service West

Integrated Discharge Service

Post Hospital Review Team

37 hours per week | Permanent

Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.

We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.

About the role

  • Working collaboratively with other partners and teams within Adult Care Services
  • Developing innovative support plans
  • Promote and support strength based assessment and practice approach to all interventions to maximise independence, health and well being to prevent or reduce the need for Adult Social Care
  • Working creatively with residents to ensure their safety and independence
  • Raising awareness in the area and developing valuable links for the team
  • Supporting other Therapists and Community Care Officers
  • Provide information and advice to enable an individual to confidently and safely participate in their daily life, includes grading of tasks and activities and adapting their environments
  • You will undertake a specialism eg Moving and Handling, BiA, AMP or Occupational Therapy practice educator


About you

Essential:

  • HCPC Registered Occupational Therapist
  • Full driving licence and regular access to a vehicle
  • 2 years post qualifying experience as an Occupational Therapist
  • Knowledge and experience in social care sector
  • Experience of completing assessments and making recommendations
  • Ability to work on your own as well as part of a team

Desirable:

  • Knowledge of Care Act 2014
  • Knowledge of Mental Capacity Act 2005
