About the Service and Team

Summary of the Cheam Locality Social Work Team

The Cheam Locality Social Work Team is a statutory service within the London Borough of Sutton Adult Social Care, supporting adults aged 18 and over with care needs related to physical disabilities, Dementia, age-related frailty, or long-term conditions.

The team's primary objectives include:

  • Conducting strengths-based Care Act assessments and developing person-centred support plans.
  • Managing safeguarding enquiries and complex casework through effective social work interventions.
  • Ensuring legal compliance with the Care Act 2014 and the Mental Capacity Act 2005.
  • Collaborating with health, housing, and voluntary partners to provide coordinated support

The Role

As a Social Worker within our Cheam Locality Team, you will manage a diverse and rewarding caseload, empowering adults with various care and support needs to achieve the goals that matter most to them. In this dynamic environment, your key responsibilities will include:

  • Conducting strengths-based Care Act assessments, reviews, and support planning in close collaboration with residents, carers, and partner agencies.

  • Leading on complex casework, including safeguarding enquiries, Mental Capacity Act assessments, and best-interest decision-making with a focus on effective risk management.

  • Cultivating strong relationships with families, health professionals, and community partners to craft person-centred solutions that promote autonomy.

  • Balancing statutory duties with preventative practice to help residents access local resources and reduce reliance on long-term formal services.

  • Ensuring high standards of case recording while actively contributing to a supportive team culture through reflective practice and continuous professional growth.

Candidate Profile

We are seeking a motivated and experienced Social Worker with a passion for delivering exceptional, person-centred care. You should be a collaborative professional, confident in making critical decisions within complex frameworks, and dedicated to achieving the best possible outcomes for our community.

Essential Criteria

You must hold a Social Work qualification with current Social Work England registration. A deep understanding of the Care Act 2014, the Mental Capacity Act 2005, and safeguarding principles is vital. Your background will include managing varied caseloads and working effectively alongside partner agencies. Excellent communication, analytical, and prioritisation skills are essential to maintaining the high quality of our service delivery.

Desirable

Professional experience in Adult Social Care—specifically with older adults or individuals with physical disabilities and conditions such as Dementia—is highly desirable. We also value expertise in court work, Continuing Healthcare, complex safeguarding, or practice education.
