All about the Role

As a Family Hubs Practitioner, you'll play a vital role in supporting children and families across the community, delivering both 1:1 and group-based interventions in a range of settings, including family homes, outreach venues and Family Hubs.

Working with families across the 0–11 age range, you will provide early help and practical support to improve outcomes in areas such as child development, emotional wellbeing, parenting skills and school readiness.

Using a whole-family approach, you will build trusted relationships, identify needs and empower families to make positive, sustainable changes.

Key responsibilities include:

  • Delivering targeted support through 1:1 sessions, parenting programmes and group work
  • Completing Early Help Assessments and leading Team Around the Family (TAF) meetings
  • Supporting child development and promoting positive parenting practices
  • Delivering evidence-based interventions to address social, emotional and behavioural needs
  • Working collaboratively with schools, health partners and other agencies
  • Encouraging families to access digital services, community resources and peer support networks
  • Ensuring the voice of the child is central to all planning and support

This is a flexible role, with occasional evening and weekend work to meet the needs of families. Please Note: This role is fully office based.

All about our Team

You'll be joining a supportive and collaborative Family Hubs team within Children's Services, committed to improving outcomes for children and families through early intervention and prevention.

We take a multi-agency, whole-family approach, working closely with:

  • Health Visitors and Midwives
  • Schools and Early Years providers
  • Other Children's Services teams and community partners

Our team is passionate about delivering joined-up, inclusive services, ensuring families receive the right support at the right time. We also place strong emphasis on co-design, involving families in shaping the services they use.

All about You

We're looking for someone who is passionate about making a difference and supporting families to thrive.

You will bring:

  • A strong understanding of child development, parenting and safeguarding
  • Experience working with children, young people and families, particularly those who may be vulnerable
  • The ability to build trusting relationships and communicate effectively in sensitive situations
  • Confidence working independently and as part of a multi-agency team
  • Strong organisational and record-keeping skills

You'll also demonstrate:

A Level 3 qualification in childcare, health & social care (or equivalent experience) is essential, alongside a commitment to ongoing development.
