The Role:

Worcestershire County Council is committed to supporting its residents to live healthy and independent lives in their community.

Our approach is to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We will continue to focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible.

This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life, and remain independent for as long as possible.


The Community Reablement Team supports individuals living in the community who are experiencing difficulties with day-to-day activities. We are often the first point of contact for people who have not previously received care or support services. Our focus is on promoting and maximising independence wherever possible, while assessing and identifying any eligible care and support needs. Through a person-centred approach, we work alongside individuals to help them regain skills, build confidence, and achieve the best possible outcomes for independent living.

We are looking to recruit to the position of Service Assessor within the Community Reablement Team, you must be committed to:

  • Undertake initial assessments within the community, identifying needs, risks, and the service user's potential for regaining independence with personal and / or domestic activities of daily living.
  • Work closely with a small team of therapists and therapy assistants and wider stakeholders
  • Develop Promoting Independence Plans that capture the desired service user outcomes and detail the required intervention of the service.
  • Actively complete regular reviews of their progress, to support with flow and capacity within the service.
  • Ensure that all standards and performance measures are adhered to, including service user monitoring protocols and quality assurance audit completion.
  • Ability to be able to challenge and confidently explain processes.
  • Excellent recording skills on IT systems.
  • Partake in triaging of new referrals and support to identify those that are appropriate

As the ideal candidate you must have a level 3 Diploma in Health and Social Care or considerable experience of working in a social care setting. It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate, and empathetic.

In return you will be supported in your role by your line manager through one-to-one meetings and performance reviews, team meetings and other corporate well being tools. You will be encouraged and supported to complete any formal accredited qualifications, and you will have a personal development plan to support you in developing towards any personal career aspirations. You will also receive 5 weeks annual leave in addition to Bank Holidays, rising to 6 weeks after 5 years’ service, and access to the Local Government Pension Scheme.
